Candidate Information

This training is open to North Dakota Firefighters and associate members. The candidate must have at least 3 years’ experience as a member of an organized fire department. All candidates must hold current certifications for Hazardous Materials Awareness & Operations, Fire Fighter I, Fire Fighter II, and Fire Instructor I in order to complete the Fire Officer I certification process. Please confirm you have uploaded your current certification documents to the NDFA member portal before completing this request form.

Authority Having Jurisdiction (AHJ) Contact Info

Please submit information for your AHJ point of contact. This individual will review each of your Job Performance Requirement (JPR) assignments to ensure department SOG/SOPs are followed. A signature from your AHJ is required on each JPR submission.
